How do I get an official passport?

Applicants are required to have approved funded orders or memorandum in lieu of orders that has been approved by the department of state in order to obtain an Official/No-Fee Passport. Orders must state a country that requires a no-fee passport by the electronic clearance guide: https://www.fcg.pentagon.mil/ 

FIRST TIME APPLICANT’S STEPS TO APPLY: Bring to Appointment 

1. Passport Application: Fill out DS-11 for first time applicants https://travel.state.gov/content/travel/en/passports/need-passport/apply-in-person.html, Passport Applications must be completed electronically using the online form filler tool. Do not sign until instructed to do so by a Passport Acceptance Agent.  

2. Evidence of U.S. Citizenship: You must present one of the following documents and provide a front and back copy                                  EVIDENCE MUST BE AN ORIGINAL OR CERTIFIED PHYSICAL COPY (ONLY ONE IS REQUIRED) 

  • U.S. Birth certificate that meets the following requirements: 

  • Issued by the city, county, or state of birth 

  • Lists your full name, date of birth, and place of birth 

  • Lists your Parent(s) full names 

  • Has the date filed with registrars’ office (must be within one year of birth) 

  • Has the registrars signature 

  • Has the seal of the issuing authority 

  • Consular Report of Birth Abroad or certification of birth 

  • Certificate of naturalization 

  • Certificate of citizenship 

3. Photo Identification: You must present one of the following identification documents and provide a front and back copy.                                                                                             

***EACH APPLICATION MUCH HAVE A FRONT AND BACK COPY OF THE SPONSORS MILITARY ID*** 

  • U.S. military or military dependent ID 

  • In-state, fully valid driver's license, or enhanced driver's license with photo 

  • Government employee ID (city, county, state, or federal) 

4. Passport Photo: (2) 2” X 2” photos https://travel.state.gov/content/travel/en/passports/how-apply/photos.html 

  • Photo Tips 

  • Photo must be taken in last 6 months in color 

  • Use a clear image of your face. No filters 

  • Remove eyeglasses for your photo 

  • Use a white background without shadows, texture or lines 

5. Official Travel Orders: Examples include  

  • Permanent Change of Station (PCS) orders 

  • TAD/DD form 1610 orders  

  • Unit Deployment Waivers

2. Most recent U.S. Passport:  

  • You must have the passport in your possessions, it cannot be damaged, you must have been age 16 or older when the passport was issued, and it must have been issued in your current name or you must show legal proof of your name change.  

  • Passport must be issued within the last 15 years 

 

3. Identification: You must present one of the following identification documents and provide a front and back copy.                                 

  • U.S. military or military dependent ID 

  • In-state, fully valid driver's license, or enhanced driver's license with photo 

  • Government employee ID (city, county, state, or federal)

FREQUENTLY ASKED QUESTIONS:

  • Applying for a child under 16: All children under the age of 16 must apply for a passport in person with two parents or guardians using form DS-11. You cannot renew your child’s passport using form DS-82. 

  • If only one Parent is available to apply: The parent who appears in the person must submit documentation to explain the absense of the other paretn. Once Parent may sign the application if he/she can provide ONE of the following 

-Notarized, signed statement (Ds-3053) from the absent parent consenting to the issuance of a passport for the minor. 

-Childs certified birth record (US or Foreign) which lists only the name of the parent submitting the application for the minor.  

-A court order granting sole custody to the parent who is submitting the application, and which does not contain restrictions on the minor's travel.  

-An adoption decree which lists the name of the parent submitting the application. 

-A judicial declaration of incompetence of the absent parent. 

-Death certificate of the other parent 

-A copy of the commitment order or other comparable documents for the incarcerated parent. 

If none of the above documentation is available, the parent who is applying for the passport must submit a non-notarized signed statement explaining why he/she cannot obtain the consent of the absent parent. 

  • Previous passport lost or stolen: Report your valid U.S. passport lost or stolen immediately to protect yourself from identity theft. Do not report your most recent passport as lost or stolen if the passport has expired. When you report your passport lost or stolen, we will immediately cancel it. You cannot use it for international travel even if you find it later. Reporting your passport lost or stolen does not automatically replace your passport. If you want a new passport after reporting your current passport lost or stolen, apply in person and submit Form DS-11.  

  • All applicants regardless of age must appear in person with proper identification: Public Law 106-113 requires that either parents or legal guardians appear in person before a passport acceptance egent and both must, under oath sign the passport application for a while under the age of 16. There will, however, be circumstances when only one parent or no parent is available to apply for the child. 

 

  • Expedited Process: 

  • When Orders are unavailable: DOD members/Units may request official Passports using a Memorandum in Lieu of Orders (MILO)/Deployment Waiver. It’s the applicant/Units responsibility to create the letter in naval formatting (and send to PSD). The Passport Services Division (PSD) handles the vetting of all official Passport requests for mission/assignments tasked where official travel orders may not be available. PSD only approves requests within 45 days of the required travel date, 60 days if the country requires a Visa. The memorandum must have significant justification information on the travel tasking requiring the bona fide need for an official passport. Why orders are not available, or able to be issued in time for travel must be included on the requests. All requests must be signed by an O-6 or equivalent form thje organization that is requesting the MILO. PSD does not accept documents signed “for”, or “By direction”. Rank/Grade should be included in signature block. Additional supporting documentation must be included with each request.
